Terms and Conditions – Repair/Disassembly Authorization – Direction to Pay
Limited Power of Attorney
By signing this form, I hereby authorize the repair and/or disassembly for damage evaluation of the above referenced vehicle. GARRISS BODY SHOP, LLC will not be held responsible for loss, or damages to vehicles or articles left in vehicles in case of fire, theft, or any other cause beyond their control. I understand that GARRISS BODY SHOP, LLC, cannot guarantee an exact delivery date as these dates are estimated. GARRISS BODY SHOP, LLC is not responsible for delays caused by the unavailability of parts or delays due to hidden damages.
The total amount of the repair charges must be paid before the above referenced vehicle can be released. Any insurance deductible amount must be paid in full before this vehicle can be released.
I hereby grant GARRISS BODY SHOP, LLC, its employees', and sublet vendors permission to operate the vehicle herein described on street, highway, or elsewhere for the purpose of testing, inspection, or to transport to/from sublet vendors/dealership service departments for sublet repairs or diagnostics`.
An express mechanic's lien is hereby acknowledged for the above referenced vehicle should the need occur, to secure the amount of any remaining balance.
Additionally, I hereby assign GARRISS BODY SHOP, LLC the right to collect and retain all amounts required to repair the above referenced vehicle, including but not limited to amounts due for labor rate discrepancies, parts price discrepancies, and repair procedures from my insurance company. This includes collection and deposit of checks made out directly to GARRISS BODY SHOP, LLC, and checks made jointly to GARRISS BODY SHOP, LLC and the owner/driver for this vehicle. GARRIS BODY SHOP, LLC may deposit the proceeds into GARRIS BODY SHOP, LLC's account with any banks, trust companies, savings banks, savings and loan associations or other financial institution(s), wherever located that GARRISS BODY SHOP, LLC deems proper. Consideration for the authorization, release, and relinquishment of funds includes, but is not limited to the repairs of the identified vehicle.
GARRISS BODY SHOP, LLC is authorized to sign or endorse in my name and on my behalf, and negotiate all checks, warrants, drafts, bills of exchange, promissory notes, letters of credit, notes or other negotiable instruments payable to me or my order, or which may require my endorsement, related to any claims or settlement involving damages and/or repairs to the above identified vehicle.
All auto body refinishing and paint has a lifetime warranty with customers original receipt for as long as the customer owns the vehicle unless otherwise noted on the repair invoice. All wheel repairs have a 12-month warranty. All interior repairs have a six-month warranty. I understand that windshields may crack during a windshield repair process and that any additional cost for replacement of windshield would be the responsibility of the driver/owner signed below.
